Apply for your Studio or Wholesale Account In 3 Easy Steps:
1) Please provide one of the following, depending on your state: Federal Tax ID, EIN Certificate, State ID#, Sellers Permit, Resale license, Tax Exempt form, or business Website. Email a copy of this proof of business to dance@blacksapphiredesign.com with any sensitive information removed. **Applications will be denied if no proof is provided**
*For Florida-based business, this document must be your Annual Resale Certificate for tax exemption; or for non-profit organizations, the Consumer’s Certificate of Exemption.
2) Fill out this form:

3) Watch for an email from “dance@blacksapphiredesign.com” within 2 business days after sending documentation, and add that email to your contacts so you don’t miss important information. That email will have information on how to order from the website, along with detailed Terms and Conditions. If you do not see an email from us, please check your spam folder or promotions tab.
